The document marked 'Registration Form' is required by the City of Sacramento for use of the park. Please sign the document at the bottom and return with the training agreement. Do not complete the rest of the fields.

PAYMENT

Please make checks payable to '4Paws University'

• The price of the Puppy Kindergarten class is $145.00 for 8 lessons.
  
• If you paid by credit card you should have received a separate email with receipt of your payment. If you did not receive an email confirmation of your credit card payment, please click here to send an email regarding your payment.
  
• If you would like to pay by Visa or Mastercard, please click here.
  
• If you are paying via check please mail your check with the Training Agreement to the address above or bring to the training site on any Saturday between 9:00 and 1:30.

Your payment must be received no more than 30 days after registration to secure your place in the class or your place will be given to students on the waiting list. 

ORIENTATION

The orientation is for humans only. Do not bring your dog to the first class. This includes toy breeds in carriers.

• The Orientation class begins promptly at 11:00 am regardless of your regular class start time and is mandatory. Your regular class start time can be found on your registration confirmation.
  
• Please check in at the front desk to finalize your enrollment
  
• Please arrive early to avoid disruption to other students.

WHAT TO BRING

• Training Agreement and Waiver (see instructions above)
  
• Proof of curent vaccinations including Bordatella (Kennel Cough). Please note, Bordatella is not part of the standard puppy vaccinations, you may have to specifically request it from your vet.

You will receive instructions regarding necessary equipment at the Orientation.

CLASS LOCATION

Our class location changes twice each year. Please double-check the directions online the day of class to ensure that you arrive at the correct location.
